BSI NUMBER 

The BSI number is the number assigned to each order for internal tracking. On an Order Acknowledgement it is referred to as a Sales Order Number. For a systems order the number will be a 100,000 number. For all other products the Sales Order Number will be in the 300,000. On a shipper its reffered to as a packing slip no.

 

CONTACT NAME 

This is the person name that has the defective product that we should contact with any questions, or the person we should contact to schedule the repair with or the person that we should see if we need to pick the product up.

 

PICKUP ADDRESS 1 

This is the street address of where we should come to repair or pick the product up if we need to do the repair in the manufacturing shop.

 

PICKUP ADDRESS 2 

This would be used to give us a room number of where the product would be found within the building. Could be a floor, room, department etc.

 

PICKUP CITY 

This is the city of where the product that needs repair is at.

 

PICKUP ZIP CODE 

This is the zip code of where the product that needs repair is at.

 

PICKUP PHONE NUMBER 

This is the phone number of the person in the contact name field. This will be the number we will call to schedule the onsite repair or to schedule pickup of the product to be repaired in the manufacturing facility.

 

PICKUP EMAIL ADDRESS 

This is the email address of the person in the contact name field. This is the email address we will correspond with if we do not make a phone call to schedule the onsite repair or schedule pickup to be repaired in the manufacturing facility.

 

QTY NEEDING REPAIR 

This is the quanitity of the product that needs to be repaired. The quanitity should be equal to or less then the quanitity ordered.

 

DESCRIPTION OF FABRIC, STAIN, LAM 

This is the information found on your acknowledgement or shipper under the part number description.

 

DESCRIBE PROBLEM 

Explain in detail what the problem is and what needs to be repaired.

 

SPECIAL INSTRUCTIONS 

Place here other information that will allow us expedite this repair request. If you need a replacement product while your product is being repaired please let us know here. If you need to be notified prior to us coming to repair onsite or to pick up the product please indicate that here.
